AS AN ELIGIBLE MEMBER OF ICWP, THE FOLLOWING SERVICES ARE PROVIDED AFTER CHOOSING A CASE MANAGER TO ASSIST IN GUIDING CARE:
PERSONAL EMERGENCY RESPONSE SYSTEM (PERS)
ICWP provides PERS is an electronic device that enables high risk clients secure help in the event of an emergency. PERS providers provide two-way verbal and electronic communication systems with a central monitoring station seven (7) days a week, 24 hours a day to geographically and socially isolated clients.
COUNSELING
ICWP provides individual counseling services assistance to clients with developmental or physical disabilities and their families to understand their capabilities and limitations or assist in the alleviation of problems of adjustment and interpersonal relationships.
ENVIRONMENTAL MODIFICATION
Environmental modifications services provide physical adaptations to the private home( No Rentals) specified in the Individual Plan of Care, which are necessary to ensure the health, welfare, and safety of the member, or which enable the member to function with greater independence in the home and without which, the waiver participants would be at risk of institutionalization.
TRADITIONAL CASE MANAGEMENT AND ENHANCED CASE MANAGEMENT
provides an assessment of the ICWP client, including diagnosis and personal history, case management issues and dynamics and a plan to address the clients needs and any unresolved issues. In addition to having knowledge and understanding of human interaction dynamics and interpersonal skills, ICWP clients will be provided with comprehensive knowledge of community resources and services needed to maintain ICWP clients in the community; access points to link clients to services and resources; and maintaining ongoing relationships with agencies and resources which are instrumental to the client’s care and service provision.
ADULT DAY SERVICES
Adult Day services provide services specializing in treatment techniques for members with traumatic brain injuries and or a major neurological deficit. Adult Day Services develop and provide services which focus on the needs of individuals with a traumatic brain injury or neurological deficit identified in the Plan of care, and the specific manner in which this service will meet the client's individual needs. The program focuses on adaptive skills and is distinct from work production objectives
PERSONAL SUPPORT SERVICES
Provide personal care assistance to achieve of activities of daily living; such as eating, bathing, dressing, personal hygiene, preparation of meals and light housekeeping tasks
FINANCIAL SUPPORT SERVICES
Services provided to ensure consumers’ funds in the Plan of Care of managed and distributed as intended
BEHAVIOR MANAGEMENT
Services that offer individualized interventions designed to decrease maladaptive behaviors
SKILLED NURSING
Treatment and healthcare procedures ordered by a Physician to provide care within the scope of nursing such dietary practices, sanitation, and use of emergency medical services.
RESPITE CARE
Temporarily relief for individual(s) that normally provide care of to an ICWP member
VEHICLE ADAPTATIONS
This service is provided for the ICWP client’s privately owned vehicle and may include hydraulic lifts ramps, special seats, and other interior vehicle modifications or devices to allow for access into and out of the vehicle, for driving the vehicle if appropriate, and for security while the vehicle is moving.
